To order by fax 24 hours a day, fax print out the order form below and fax to (954)345-0671. Please be sure to include your e-mail address or telephone number so we may reach you if we have any questions about your order.

Please be sure to include a copy of your order with your check or money order. Also be sure to include your mailing address and your e-mail address. You will be notified by e-mail when your order is being shipped. We do have a 7 business day waiting period for check clearing. Payment by money order allows for shipping within 3 business days.

We accept Visa, Mastercard, and American Express. You may provide your credit card information either electronically via this order form. NOTE: AT THE PRESENT TIME, THIS IS NOT A SECURED OR ENCRYPTED TRANSACTION. You may also place a credit card order by phone, fax, e-mail, or snail mail.

Please be sure to provide the following information: Name & Address as it appears on the card & billing statement, Card #, Expiration Date.

Payment by credit card allows for shipping within 3 business days. If you mail or fax your credit card information, please be sure to include your order information, your shipping address and also your e-mail address. You will be notified by e-mail when your order has been shipped.

Shipping/Insurance/Handling
We use USPS Priority Mail for all shipments unless otherwise requested by our customer. Selecting other methods of shipping may increase your cost. Shipping charges listed below are for shipments within the continental US. Shipping charges for orders outside the continental US & international orders will be based upon the service selected by our customer.

    1st Item: $6.95
    Each Add’l Item: $2.00 (shipped at same time to same US address)

There may be times when due to unusual weight or value, shipping charges will differ from the above schedule. Shipping charges will be disclosed on your order confirmation.

Deposit Information
All deposits placed on pre-ordered items are non-refundable. The balance is due within 10 days of being notified that your item(s) are in.

Return Policy
If you are not completely satifsied with your purchase, please notify us within 7 days of receipt of your order. We will gladly offer an exchange or refund of your purchase price. All returns must be pre-authorized in advance. No returns on pre-ordered items unless damaged in shipment. Items in "De-Boxer’s Delight" are sold as is and are final sales. Please notify us immediately if your order was damaged in shipment. We will initiate the claim process immediately, as all packages are shipped with insurance.
